Winners for the above categories, you will have thirty (30) days from posting to claim your prize. If the thirty (30) day period has pasted and you have not claimed your winnings, they are then forfeited.
How to claim your winnings!
If you are a winner, you must fill out the form below and hit the submit button. If you have won a cash prize, you must enter your name & full address to receive a check made payable to you. The form must be filled out completely in order to receive your winnings. Your e-mail address needs to be the same as when you sent your winning contribution in order to receive your winnings. Thank you and congratulations. All cash prizes will be distributed within 30 days of notification.
